The MaxLife Church performs Baby Dedications for children ages 0 - 8 years of age. Children ages 8 & up will be baptized.
Dedications ceremonies are generally performed during our 9:45 AM worship service.  If you desire to have your child/children dedicated, kindly submit your request by completing the form below.  Please provide (2) two dates. Your first date should be your first choice.   

All dates must be submitted (2) two weeks prior to the first date you choose.  The Administrative office will review both dates and confirm the Pastor’s availability. Once you date has been approved, you will be notified. At that time, you will need to fax the candidate(s) birth certificate to the Administrative office. This information must be received at least (7) seven business days prior to the date of ceremony.
